The Right Technical Director
Instrumental for Ensuring Quality in The Echocardiography Laboratory

Serving as Technical Director in more than one Laboratory

After The ICAEL Standards were amended to require a credentialed Technical Director effective January 1, 2007, it unfortunately became apparent that some laboratories tried to circumvent this requirement by naming a sonographer who was credentialed, but had little or no connection with the laboratory. It is the intention of The Standards that the Technical Director is a full-time employee dedicated to the technical leadership of the laboratory. Therefore, if a Technical Director is also the Technical Director at another laboratory, then there must be a second credentialed sonographer employed in the laboratory, who will be present in the laboratory and will take over the duties of the Technical Director when the primary Technical Director is out of the laboratory.

Under some circumstances, a sonographer may be the Technical Director for more than one laboratory and working full-time in each, in accordance with The ICAEL Standards. This may occur, for example, when a sonographer is the Technical Director of two small offices, one open only Monday, Wednesday and Friday, and the other open only Tuesday and Thursday. Even though that individual does not work all week in either office, he or she still works 100% of the time that each office is open. In this situation, the applicant laboratory should provide an explanation to clarify that it is upholding the intention of The Standards.

Changes in the Technical Director Position in a Laboratory

A laboratory that has submitted an application or is accredited must notify the ICAEL within 30 days of the date of the departure of the individual serving as the Technical Director. An Affidavit Approving Change in Operations can be downloaded from the ICAEL website at www.icael.org/icael/apply/affidavit_operations.doc. This document may be used as notification; laboratories will be contacted if additional information is needed. A new Technical Director must be in place, and all requested documentation submitted, within 60 days of the departure date of the previous director.

A Technical Director should be chosen prudently. He or she is a valuable asset to the laboratory in earning ICAEL accreditation, upholding compliance with The ICAEL Standards and ultimately ensuring the quality of care provided to patients.

One-Year Provisional Grant

First-time applicant laboratories without a credentialed Technical Director (or second credentialed sonographer if required) may take advantage of the “provisional grant” decision pathway, which recognizes that laboratories are moving forward but have not yet achieved full compliance in three areas of The ICAEL Standards:

  1. Adequate quality assurance
  2. Sufficient Continuing Medical Education credits
  3. A credentialed Technical Director

The notice of a provisional grant advises laboratories, referring physicians, payers and patients that accreditation has been awarded for one year rather than three years, despite lack of compliance in one or more of the above three areas. The laboratory without a qualified Technical Director must demonstrate compliance with The Standards (as stated in Part I, Section 1.2.1) before the end of that one-year period in order to be converted to the [remainder of the] three-year grant. Otherwise, the accreditation will lapse until appropriate documentation of a credentialed Technical Director is presented.
