REPRESENTATIVE CASE STUDIES

The purpose of reviewing representative cases is to evaluate the interpretive and technical quality of the laboratory as a whole. In order to do this, it is necessary for the ICACTL reviewers to evaluate representative cases from as many medical and technical staff members interpreting or performing any CT examinations in the laboratory as possible. The cases submitted must be contemporary (i.e., performed with current personnel on current equipment). Failure to provide representative cases from the current personnel may result in delay of accreditation and request for additional case studies.

The number of cases to be submitted per laboratory is dependent upon the type of accreditation requested (i.e.: Cardiovascular, Neuroimaging, other CTA, Whole Body CT) and the number of CT scanners within the laboratory. All case studies selected must have been performed within the twelve (12) months immediately prior to submission of the application. Each case must include a sufficient number of images to support the final diagnosis and a signed copy of the final report. A copy of any worksheets and/or preliminary reports, if generated, should be submitted as well.

The following information must be on or included with all submitted CDs, DVDs or filmed images:

  • laboratory name
  • date of the examination
  • patient age or date of birth
  • slice thickness
  • patient ID
  • field of view
  • acquisition
  • size scale

Guidelines for case selection and organizing cases to be submitted:

1. Case selection is CT scanner dependent and lab specific. You must submit the required sample cases for each scanner used clinically in your laboratory.

2. The case studies must include a copy of the final report and be representative of as many medical and technical staff members interpreting or performing any CT examinations in the laboratory as possible.

3. The case studies must be submitted by film, CD, or DVD* (with viewer or reader installed) for those areas in which accreditation is being requested . (Please check the CD or DVD on an alternate computer to ensure that it can be opened and viewed.)
*Comment: Cardiac case studies and all CTA studies are to be submitted on CD or DVD only, with viewer or reader installed.

4. If your laboratory does not perform all of the case studies required as outlined, please submit those meeting the requirements, as well as additional studies relevant to your laboratory's practice, to equal five (5) cases total.

5. All hard copy documents including CDs and DVDs must be labeled with a unique patient ID (i.e.: patient name, laboratory tracking number, etc.), the type of examination and the type of hard copy
being submitted. Do not submit black and white photocopied images.

6. As explained in the General Instructions, two complete copies of the application AND representative cases are to be submitted. All hard copy documents, reports, films, CDs, DVDs, etc., are to be submitted in duplicate as well.

Note: All material submitted to the ICACTL will be handled with strict confidentiality.

CASE STUDY REQUIREMENTS:

  • CORONARY CALCIUM SCORING ACCREDITATION

    Case One: 1 normal scan
    Case Two: 1 abnormal demonstrating mildly elevated calcium score
    Case Three: 1 abnormal demonstrating severely elevated calcium score
    Case Four: 1 abnormal demonstrating valvular/annular calcification
    Case Five: 1 abnormal demonstrating extra coronary pathology


  • CARDIOVASCULAR CTA ACCREDITATION
    To be submitted on CD or DVD, with viewer installed and instructions as necessary; include only those phases reconstructed, indicate phase(s) and field of view reconstructions used for interpretation.

Case One: 1 complete gated cine cardiac examination in a normal patient
Case Two: 1 cardiac examination in a patient with non-obstructive CAD
Case Three: 1 cardiac examination in a patient with obstructive CAD
Case Four: 1 complete gated cine cardiac examination in a patient with LV dysfunction secondary to myocardial infarction
Case Five: 1 cardiac examination in a patient with prior coronary bypass graft


  • OTHER CTA
    To be submitted on CD or DVD only with viewer installed.

    Case One: 1 extracranial carotid artery axial, coronal, sagittal, and 3-D reconstructed views -- 1-2 mm cuts
    Case Two: 1 arch, descending thoracic, and abdominal aortic imaging axial, coronal, sagittal, and 3-D reconstructed views -- 1-2 mm cuts
    Case Three: 1 endovascular repair of the aortic arch, descending thoracic, and abdominal aorta. Axial, coronal, sagittal, and 3-D reconstructions 1-2 mm, pre-contrast, arterial and venous phase. Include any additional CT imaging performed.
    Case Four: 1 lower extremity imaging (i.e.: femoral-ankle) arteriogram, with axial, coronal, sagittal, and 3-D views -- 1-2 mm cuts.
    Case Five: 1 vascular study of choice


  • NEUROIMAGING CT ACCREDITATION

BRAIN ONLY
Case One: 1 brain - stroke or bleed
Case Two: 1 brain - normal
Case Three: 1 brain - tumor
Case Four: 1 brain - trauma
Case Five: 1 brain of choice

BRAIN AND SPINE
Case One: 1 brain - stroke or bleed
Case Two: 1 brain - normal
Case Three: 1 brain - tumor
Case Four: 1 spine - abnormal
Case Five: 1 spine - normal


  • WHOLE BODY CT

Case One: 1 abdomen (i.e.: liver or kidney or pancreas)
Case Two: 1 pelvis (i.e.: ovaries or prostate)
Case Three: 1 chest (non-cardiac)
Case Four: 1 extremity case with reformats in plane according to lab protocol
Case Five: 1 study of choice


  • SINUS AND TEMPORAL BONE CT
    Choose a total of 5 representative studies from the following categories that are relevant to the lab. If both sinus and temporal bone CTs are performed, choose a representative mix.

SINUS
Case One: 1 demonstrating tumor
Case Two: 1 chronic sinusitis
Case Three: 1 demonstrating a deviated nasal septum
Case Four: 1 facial pain or headache
Case Five: 1 normal

Other pathology to be submitted if cases not available from list above:
-Nasal obstruction
-Polyposis
-Anosmia
-Chronic posterior epistaxis

TEMPORAL BONE
Case One: 1 tumor
Case Two: 1 chronic mastoiditis
Case Three: 1 cholesteatoma
Case Four: 1 hearing loss (non-MRI pt)
Case Five: 1 normal

NECK
1 study of choice
