The Accreditation Agreement is now available as a download from the ICAVL website. The ICAVL has provided this downloadable format as a service to laboratories.

Please note that the Accreditation Agreement contains blank areas for laboratories to enter their personal information. Users may ONLY enter information into the specified areas provided. The surrounding text of the Agreement may not be altered by any means without express written permission from the ICAVL. Attempts to alter any part of the Agreement text without permission from the ICAVL automatically voids the Agreement, and will be cause for civil liability.
