At this time, there is not a questionnaire in the Online Accreditation application to complete when adding a multiple site (fixed and/or mobile) to an already accredited laboratory.
Accredited laboratories wanting to add a multiple site (fixed and/or mobile) must submit the completed Multiple Site (Fixed and/or Mobile) Supplemental Application via trackable mail. If there are any physicians and/or technologists at the additional site that were not included as part of the original application, it is necessary to complete the necessary staff and equipment forms located in the Changes Within the Lab section. ALL of the forms included in the Multiple Site (Fixed and/or Mobile) Supplemental Application, available for download above, including the Affidavit of Change in Ownership or Operations and the necessary staff forms must be submitted. Sites are not considered accredited until the documents have been reviewed, fees have been submitted and accreditation has been granted.
It is critical that you access your lab's IAC Online Accreditation account as soon as possible to enter and/or verify the new site and update the National Provider Identifier (NPI) data and Medicare enrollment numbers. CMS as well as other private insurers will verify the accreditation status of a supplier only through data file transfers containing this information. Failure to enter accurate identification data will result in nonpayment of Medicare claims after 1/1/2012. Upon receipt of all required materials, the accreditation decision will be made at the next monthly IAC Board of Director's meeting. If the added multiple site (fixed and/or mobile) is accepted into the laboratory's accreditation, accreditation for the added multiple site (fixed and/or mobile) will expire when the laboratory's original accreditation expires.

Effective 3/24/2011: The IAC’s accreditation policy related to mobile services has changed. Effective immediately, the policy previously defined as Mobile Services has been eliminated and the Multiple Site policy is now defined as Multiple Sites (Fixed and/or Mobile).
