Document Policy (please read prior to download)

By selecting to download the Agreement document, you hereby agree to the terms/policy stated below.

1. The Accreditation Agreement contains blank areas for laboratories to enter their personal information. Users may ONLY enter information into the specified areas provided. The surrounding text may not be altered by any means without express written permission from the IAC. Attempts to alter any part of the agreement text without permission from the IAC automatically voids the agreement, and will be cause for civil liability.

2. The Accreditation Agreement with track changes turned on, is ONLY for use by labs that have selected to modify the wording of the Agreement. These labs have agreed to paying a $200 legal fee and will have a legal representative make changes of the downloaded copy of the agreement (with "track Changes" turned on). Labs will need to attach a copy of the Agreement with the requested changes to an email and send to iac-agreement-help@intersocietal.org. The IAC attorney will review and accept the changes or include alternative language that is acceptable and will ask you to print, sign and mail the Agreement with original signatures, to the IAC office.
