When to Submit Your Final Application

First-Time Applicants: Applicant facilities may submit their completed online applications when the pre-submission case check has been completed. It is highly recommended that the online application and hard copy materials are submitted in a timely manner to allow the application to be assigned to the earliest possible date of decision.

Reaccreditation Applicants: To maintain uninterrupted accreditation status, the IAC recommends that accredited facilities submit their applications at least three months prior to the expiration date*. Applications are accepted at any time and can be submitted up to six months prior to the expiration date.

*A grace period of 60 days beyond the facility’s expiration date is extended to those facilities that have submitted a completed application for reaccreditation prior to the expiration date of the current accreditation.

Mailing Your Facility’s Corresponding Documentation

The case studies, IAC Accreditation Agreement (if modified and not submitted as part of the application) and fee (if paid by check) must be received in the IAC office no later than 5 days following the online submission. Application submissions with the corresponding application materials that arrive late or are incomplete will be processed with the subsequent submission group. It is highly advised that all information be collected and sent in a single shipment via traceable carrier to: IAC Vascular Testing, 6021 University Blvd., Suite 500, Ellicott City, MD 21043.
